WIA, Title II Program Implementation Survey
Welcome to the 2012-13 Survey of WIA/AEFLA Title II 225/231 and EL Civics programs in California. Surveys are to be submitted online by Tuesday, April 30, 2013.
The California Department of Education (CDE) requires collection of this data, which allows the state to gather important qualitative and quantitative information and meet federal and state accountability requirements. Survey results help to evaluate student progress and goal attainment, document the extent of program improvement, and inform required reports for the California State Legislature and the Federal Government.
Only one person per agency can access and complete the online survey. The Program Administrator should be the one to fill out and submit this survey, but should get input from staff members in order to provide as much information as possible. Adult schools and jail programs within one district must complete a separate survey for both programs.
This survey contains 29 questions. If your agency has an EL Civics program, you will be asked to complete an additional 4 questions. The estimated amount of time needed to complete the survey is 30 to 45 minutes. It is not necessary to complete the entire survey in one sitting. You can exit the survey by clicking the Last button at the top or bottom of your current page. From the last page, you can select Review Answers and Return to Survey Later, then click the “Go” button to save your answers. Do not forget to return to the survey. Once you finish completing the survey, you must select Submit Answers as Complete, then click the “Go” button to finalize your submission. You are not in compliance until you finish this process.
All responses are confidential. Names of agencies or respondents will not be used without permission.
